Referral Received
Dr Jefferson’s Triage
Patient Questionnaire Completed
Phone Consult with Our Practice Nurse
Any Medical Therapy Trial and/or Testing you have Obtained
Our Rapid Access Clinic is held every month for patients who will likely benefit from an expedited appointment.
It is a 10-minute appointment (rather than the standard 20 minutes) to discuss surgical options.

If you do not wish to be seen in the Rapid Access Clinic, your appointment will remain on the date and time previously booked.
